すいません、いま時間がとれず、どなたかお考えいただければありがたいです。

質問したいことは各ページの上部の横線のことです。 
上記の先生の本と同じスタイル（もちろん英語と日本語の 
違いはありますが）のheadingを出したいと思っています。 
各章などtexのテキストに対応したところは望んだように 
印刷されますが、BibliographyとIndexのところが望んだ 
ようにいかず、横線がでません。Bibliographyの最後の 
ページだけが望んだようになっています。  
以下に現在用いているスタイルの最初の部分と問題の 
BibliographyとIndexの設定の箇所を記します。 
どのように修正したら横線がでるのかご教示いただけると 
大変ありがたいです。
